SWC is a state operated intermediate care facility for persons with mental retardation (ICF/MR) facility that specializes in serving individuals with developmental disabilities, often combined with mental illness and certain problem behaviors. SWC provides a wide variety of programs and services designed to assist and prepare individuals to live successfully in the setting that best meets their needs. SWC also offers specialized outreach services for people with developmental disabilities who live in the community.

Located in Southern Wisconsin near Union Grove, SWC brings together a rich history and state of the art treatment approaches for the new millennium.

Southern Center is owned and operated by the State of Wisconsin's Department of Health and Family Services, Division of Disability and Elder Services.

Mission Statement
Southern Center provides quality residential care and treatment to Wisconsin citizens with developmental disabilities when this care cannot be provided by local human service agencies.

The Center strives to prepare the individuals for integration into main stream living. The Center works individually and in cooperation with other agencies to provide support to families of persons with developmental disabilities and education and training services to area communities.

Directions
Southern Wisconsin Center is located on County Hwy C, 2 miles west of Union Grove, WI. 

From the West: Take Hwy 20 east to Hwy 45, then south 3 miles to Hwy C and go west to the North Entrance.  Or, take Hwy 11 east of Hwy 75, going 1.1 miles to Hwy C, turn north and east to the entrance.

From the East: Take Hwy 20.5 miles west of I-94 to Hwy C, then 5.5 miles west to the North Entrance.  Or, take Hwy 11 to Hwy 45, north to 10th Avenue and west 1.5 miles to the South Entrance.

From the North or South: Take Hwy 45 to either Hwy C or 10th Avenue and turn west.
